Floaters are the small dots, squiggles, or cobweb-like shapes that drift across your vision. They are caused by tiny clumps of gel or cells inside the vitreous β the clear, gel-like substance that fills the inside of your eye. Flashes of light (like a camera flash or lightning bolt at the edge of vision) occur when the vitreous tugs on the retina.
Most floaters are caused by aΒ posterior vitreous detachment (PVD) β a natural, age-related process in which the vitreous gel shrinks and pulls away from the back of the eye. PVD is very common, especially after age 50, and is usually harmless. However, it is important to have it properly evaluated, because in a small number of cases the vitreous can pull hard enough to tear the retina.
A retinal tear is a medical urgency β if not treated, it can lead to a retinal detachment and permanent vision loss. This is why any sudden new floaters or flashes should always be checked promptly.
- New floaters β especially a sudden shower of them
- Flashes of light, especially at the edges of vision
- A shadow or curtain moving across your vision
- A sudden decrease in vision
A sudden shower of new floaters or a shadow in your vision requires same-day evaluation. Please call us at (703) 841-2310 Β right away.

Dilated Retinal Exam
Dr. Smithen will dilate your eyes and carefully examine the entire retina β including the peripheral retina where tears most often occur.
Retinal Imaging
Wide-field imaging and OCT to get a detailed view of the vitreous, retina, and any areas of concern.
Laser Treatment if Needed
If a retinal tear is found, it can usually be treated immediately with a quick, painless laser procedure in the office β sealing the tear before it becomes a detachment.
Most floaters are harmless β but they all deserve a proper evaluation. Dr. Smithen will give you a thorough exam and clear answers, and will see you urgently if your symptoms suggest something more serious.
